The management of patients with locally advanced rectal cancer has improved over time, but a substantial proportion of them still succumb to their disease. Therefore, intensive, multimodal, preoperative treatment strategies using radiotherapy and chemotherapy (also known as total neoadjuvant therapy) have increasingly been used in routine clinical practice in order to reduce the risk of cancer recurrence after surgery and improve survival. However, it is debatable whether the survival benefits of using such intensive approaches are large enough to justify the increased risk of treatment-related toxicities and long-term functional sequelae, which can negatively affect quality of life. This concern is especially valid for older patients, who are generally less likely (due to their shorter life expectancy) to benefit from the small incremental survival advantage of total neoadjuvant therapy but more prone than younger patients to suffer toxicities. This clinical trial aims to demonstrate that conventional, less intensive, pre-operative treatment strategies such as radiotherapy alone offer a better trade-off between efficacy and toxicity for locally advanced rectal cancer patients aged ≥70 years, as compared with total neoadjuvant therapy. This study has the potential to establish the optimal care in this setting and to provide physicians and patients with all-round, comprehensive evidence that can be used to inform decision-making, improving overall patient experience during treatment as well as survivorship after treatment.
| Title | Efficacy and safety of short-course radiotherapy (SCRT) versus total neoadjuvant therapy in older patients with locally advanced rectal cancer: a multicentre, open-label, randomised pragmatic clinical trial – SHAPERS |
| Participants (P) | Patients ≥70 years, ECOG PS ≤1 Histologically or cytologically confirmed adenocarcinoma of the rectum Operable stage III (including involvement of lateral pelvic lymph nodes) or high-risk stage II No contraindications to the use of oxaliplatin-based chemotherapy |
| Intervention (I) | SCRT: 5 fractions of 5 Gy followed by Total Mesorectal excision (TME), with or without adjuvant chemotherapy (at the discretion of the investigator, who will also decide on type and duration of adjuvant chemotherapy). |
| Control (C) | RAPIDO: SCRT (5 fractions of 5 Gy) followed by up to 18 weeks of oxaliplatin based chemotherapy (mFOLFOX6 or CAPOX) followed by TME or watch and wait
RAPIDO Light: SCRT (5 fractions of 5 Gy) followed by up to 12 weeks of oxaliplatin based chemotherapy (mFOLFOX6 or CAPOX) followed by TME or W&W.
OPRA INCT[1]-CRT(chemoradiation therapy): up to 4 months of oxaliplatin based chemotherapy (mFOLFOX6 or CAPOX), followed by CRT (25-28 fractions of 1.8-2.0 Gy each with or without a boost to the primary tumour, for a total of 50-56 Gy of radiation, combined with either continuous infusion fluorouracil or capecitabine) followed by TME or W&W
OPRA CRT-CNCT[2]: CRT (25-28 fractions of 1.8-2.0 Gy each with or without a boost to the primary tumour, for a total of 50-56 Gy of radiation, combined with either continuous infusion fluorouracil or capecitabine) followed by up to 4 months of oxaliplatin-based chemotherapy (mFOLFOX6 or CAPOX), followed by TME or W&W.

| Outcome (O) | The primary objective of the trial is to demonstrate that SCRT, followed by surgery with or without adjuvant chemotherapy is a better trade-off between efficacy and safety than total neoadjuvant therapy in this special needs population. |
| Trial Design | This is a multicentre, open-label, randomised, pragmatic clinical trial, comparing standard-of-care neoadjuvant treatment options for older (i.e., ≥70 years) patients with high-risk stage II and stage III rectal cancer. |
